REVISED CMS-1500 Claim Form
We currently support use of both the old and the new CMS-1500 forms as well as electronic billing with NPI.
You also have the option of using pre-printed CMS-1500 forms in the new format. This method is most frequently used with Medicare claims that you choose to print yourself.
